Respiratory: N/A
Medications:
- Clonazepam (Klonopin) 0.25 mg orally twice daily, initially increased by 0.25 to 0.5
mg/day increments every 3 days according to response, maximum 4 mg/day
- Fluoxetine (Prozac) 10 mg orally (intermittent release) once daily initially, increased by
10-20 mg/day increments every 4 weeks according to response, maximum 80 mg/day
Nursing Orders:
- Assist patient with slowing the breathing.
- Provide a non-stimulating environment
- Administer medications as prescribed and assess for adverse side effects.
Follow Up Lab tests: N/A
Diagnostic testing:
- CMP: evaluation to rule in or rule out underlying cause
- CBC: evaluation to rule in or rule out underlying cause
- Troponin I & Troponin T: detection of myocardium infarction
- Drug toxicology testing, urine: screening to rule in or rule out ingestants or co-ingestants
- Blood alcohol concentration: detection of blood alcohol level to rule out withdrawal
- Thyroid function test: evaluation of thyroid function
- 12 lead ECG: evaluation of heart rhythm, detection for ST elevation or tachycardia
- Chest x-ray AP: evaluation for physiological abnormalities.
Consults: Psychiatric for assistance if additional management is needed
Patient Education and Health Promotion (address age appropriate patient education if
applicable):
- Lifestyle modification to reduce anxiety
o Identifying and removing triggers: caffeine, stimulants, nicotine, dietary triggers,
stress, smoking
o Improve sleep quality/quantity and physical activity
- Smoking cessation
- Psychotherapy: Cognitive Behavior Therapy and relaxation techniques
- Limit alcohol consumption
- Medication education: importance of adherence, side effects, potential adverse effects,
contraindications, and not to withdrawal abruptly
